Completing an About Me form and taking it to your appointment will help your healthcare professional, and you, to make treatment decisions which are best for you.

You can download the form from the Nottingham Integrated Care Board website.

Alternatively Complete the form on the Patients Know Best (PKB) website. This is a single place for all your health information and is connected to the NHS App so it  will be stored for future appointments.

These NHS England information booklets can help inform you about your treatment options for some common health conditions like knee and hip arthritis.

There are some similar support tools from versus arthritis, which cover other conditions such as back and shoulder pain.

The OK to Ask campaign helps you think about your choices and ask questions, such as:

  • What are all my options?
  • What are the benefits and risks of each option?
  • How will each option affect what is important to me?

The NHS App can allow you to access a range of NHS services including ordering repeat prescriptions, viewing you health records and booking appointments.

Sherwood Forest Hospitals are encouraging all patients to use the NHS App to securely access their personal health record.

Working with the NHS-approved platform, Patient Knows Best (PKB), you can access a range of hospital information.

Within our service, we use the NHS App and PKB to:  

  • Access a library of self-help information whilst you are waiting for a referral
  • Access and add to your B-Fit Digital Care Plan (if invited).

We recommend you register for the NHS App and PKB prior to your appointment with us.

Virtual Carers roadshow

There are around 80 videos on the virtual carers roadshow YouTube channel.

They cover a wide range of subjects. These  include, advocacy, shaping services, community grants, Healthwatch, health and wellbeing, mental health, support groups, learning disability services, dementia support and many more including videos posted by the NHS as well as the local authority.
